Location, Parking, and Arrival Information
Our hospital is located at 2134 W Division St, Chicago, IL 60622. Paid street parking is available in front of our building and there are some free parking spaces on our side streets, but be mindful of permit parking signs. Please arrive 5 minutes before your scheduled appointment.
Medical Records
If you have visited another primary veterinarian or a specialty hospital since your last visit with Companion Animal Hospital Wicker park, please let us know so we can obtain those records prior to your visit.
Punctuality and Cancellation Policy
We pride ourselves on giving clients and patients adequate time to have their questions answered and honor our appointment times. Patients who are more than 10 minutes late may be asked to reschedule for another day.
We kindly ask that you give us 24 hours’ notice if you need to cancel or reschedule any appointments so that we may make other arrangements. A deposit may be required if appointments are cancelled or rescheduled with less than 24 hours’ notice.
Payment Information
Payment is due at time of service. We accept all major credit cards, cash, checks, Care Credit, and Scratchpay.
Scratchpay
